Payment Policy

Payment for services is due at the time of the visit. I accept cash, credit card (Visa, MasterCard, American Express), and PayPal (info@fitlifewellnessllc.com).

Insurance Holders

If you have health insurance, your insurance card must be presented on the first visit. Clients are responsible for their health care charges and should be familiar with the policies of their insurance provider. Please be aware of the amount of your co-pays, which will be due at the time of your visit. If your insurance carrier changes, please notify us on your next visit.

 

 

 

Cancellation policy

I understand that we may sometimes appointments have to be rescheduled. If you need to reschedule, please call or email me (404) 807-2770 or info@fitlifewellnessllc.com at least 24-hours in advance.

If you miss an appointment or cancel without giving a 24-hours notice, you will be charged the full amount for a session.

It is very important that you are on time for your session. I respect patient schedules, so please note that there may be a session booked directly after yours. If you arrive late, your session time may be shorter. Please communicate with me as soon as you are aware that you may be late.
